Notice to new members:
If you are a new member under the Teamsters Local 213 Health & Welfare Hour Bank Plan, you must complete Plan enrolment forms and return them to the Teamsters Local 213 Members Benefit Plans office before coverage can be activated. You must complete new enrolment forms if you have had a break in coverage for more than 6 months.
These forms are mailed to your home address, as well as other plan-related information/notices, so be sure to have your mail forwarded to your temporary address while working on a project.
